Club Discount Application Form

We are offering the following discounts to our Junior & Senior members. Each player is only entitled to apply for 1 discount. The discounts are - Bring a Friend, Family Discount and Coach/Manager Discount. More information regarding these discounts are available on the form.

Criteria for Reduction in Hockey Fees

This form has the criteria and procedures required to apply for the above.

Codes of Behaviour - Players, Parents, Spectators and Coaches

Injury/Accident Report Form

This form to be completed whenever an injury occurs on the Ipswich grounds or while fulfilling representative commitments at other locations.

Umpires Performance Sheet

This form allows for feedback on an umpire's performance, both POSITIVE as well as constructive, to be submitted to the Umpires Committee.

Judiciary - Players Response Form

This form to be completed by players who have received a yellow or red card.
